Please arrive at least 15 minutes before your scheduled appointment. There are questions to answer and papers to fill out. You may print out and complete our patient registration forms in advance using the printable forms link below.
Your First Appointment
You should bring the following with you to your appointment:
- All insurance cards, information, and any neccessary referrals required by your insurance company before treatment.
- A list of medications you are currently taking, including, over-the-counter medications and herbal products.
- For your medical co-pay and/or your cosmetic appointment, we accept cash, check, or a credit card. Please note that many medical services are covered by insurance, whereas cosmetic services are elective and not covered by insurance.
During the first visit, the providers and staff will be learning about you and your concerns. To help them gather as much information as possible please ask your referring doctor’s office to have your relevant medical records forwarded to our office in time for your visit. These records include:
- Surgical reports
- Pathology reports
- Laboratory test results
- Referring doctor’s notes

Cancellation and No-Show Policy
Canceled Appointments: We understand that you may need to change or reschedule your appointment. In order to serve all of our patients in a timely fashion, you must cancel your appointment at least 24 hours in advance. If you do not provide a 24-hour notice, you will be charged a $50 fee.
Missed or No-Show Appointments: Our policy is to charge for missed or no-show appointments at the rate of $50. Please help us serve you better by keeping scheduled appointments or notifying us 24 hours in advance. If you do not provide a 24-hour notice, you will be charged a $50 fee.
We accept Care Credit as a means of financing your dermatological or cosmetic procedures.
We make every effort to keep down the cost of medical care. All co-pays, co-insurance, deductibles and payments for non-covered services are to be paid at the time the service is rendered. All cosmetic consults are required to be paid in full at the time they are performed. Cosmetic services are to be paid prior to the procedures. Please bring your insurance card with you to each appointment. Please notify our staff of any insurance change before being seen and remember that your insurance coverage is a contract between you and your insurance company. If you do not bring your insurance card, you will be responsible for charges at the time of the visit. If your insurance policy requires a referral to be seen, it is your responsibility to ensure that the referral has been received by our office. If the referral is not received by the time of your appointment, you may reschedule or sign a waiver and pay for the services. If you have any questions please feel free to ask any staff member.
